CNAS PRIZE WHEEL RENTAL REQUEST FORM
Please fill out the form below to request the CNAS prize wheel for use at your event.

Rental requests from CNAS-affiliated departments and programs are considered on a first come, first serve basis. Requests must be submitted 2 weeks prior to event start date.

The prize wheel rental package includes the wheel itself plus one whiteboard marker. Swag requests must be submitted separately using the CNAS Merchandise Request Form at https://forms.gle/y75uZJQxM7LhFVhL9.

A member of the CNAS Communications team will contact you when your request has been approved. The prize wheel can be picked up from the CNAS Dean's Office (Geology 2258) 24 hours prior to your event. The CNAS Dean's Office is open Monday through Friday from 8:00 AM - 5:00 PM.

Note: The prize wheel must be returned to the CNAS Dean's Office within 24 hours of the event conclusion, or on the first post-event business day for weekend events.
