CONFIDENTIAL MEDICAL REPORT

PROCEDURES FOR STUDENT MEDICAL EXAMINATION

1. Fill the student medical examination form online using GOOGLE FORM from the Institute’s website.

2. Visit the Laboratory Unit of the GIMPA CLINIC for your blood sample to be taken after filling the form.

3. Go back to the Laboratory for the respective results, and proceed to the OPD for procedures on weight, height, and blood pressure.

4. The OPD In-Charge will schedule your consultation with a Medical Officer for the completion of the medical examination.
