Military Recruiters' Access Opt-Out for Juniors & Seniors
FCSD is required by law to supply the names, addresses and phone numbers of all Juniors and Seniors to the Department of Defense, as it does for other post-secondary institutions. If you do not wish your child's name to be released, you must fill out this form. By placing your name and your child's name on this form, this counts as your official signature. This form must be completed by October 31, 2021. If you have more than one child who is a Junior or Senior, you need to fill out this form for each child. If you filled this form out for a student who was a junior last year, you do not need to fill this form out again. Thank you.
