JS Bus Intent Form 2022
You must complete this form whether or not your musician will be riding the bus to Lincoln Center on Sunday, May 8, 2022.

It is imperative that all musicians arrive ON TIME at WPHS or Alice Tully Hall. We will be on a tight schedule.
 
Two large coach buses will be available to transport JS musicians to Alice Tully Hall, Lincoln Center. Departing from White Plains High School's Student Parking Lot - 550 North Street, White Plains, NY 10605.

JS musicians must report for the bus at 8:30 am. Bus departs at 9:00 am.

The GWYOA will NOT provide transportation back to Westchester from Alice Tully Hall.

ALL MUSICIANS must check in with security at the stage door entrance at:
142 W 66th Street, New York, NY 10023 (between Broadway and Amsterdam)

Permission Form for Trip: Transportation will be provided this season for the following:
 - Spring Gala Concert (From White Plains High School to Lincoln Center, NYC. Return transportation NOT provided.)
In the event that there are not enough seats on the provided buses, musicians may need to be driven by chaperones or parents of fellow musicians.

I give permission for my child to participate in the above-mentioned trip.  As a parent or guardian, and on behalf of my child, I authorize that first aid or emergency medical treatment as may be deemed necessary or desirable by the person or persons in charge of the program or activity be applied in the event of an accident, injury or illness to my child.  Further, I release the Greater Westchester Youth Orchestras Association (GWYOA) and its respective officers, directors, employees, conductors, chaperones, members or agents from any and all liability for any acts or omissions out of or in connection with my child’s participation in this trip.  In the event that my child causes any bodily injury or property damage by his/her negligence, I agree to indemnify and hold harmless the GWYOA, its officers and agents from any expense or loss arising out of such negligence. *
