Please also include a printed version of this form with your shipment, or a note with your contact information so we can connect your shipment to your completed form (an email copy will send to us when you have completed this form).

For all non-warranty repairs, an inspection fee of $30 will be applied. VSE inspects incoming items and provides
estimates for work requested. VSE will complete approved work once payment is finalized.
