Celebration of Saints Form
Fill out the information below and submit it by Thursday, November 10, 2022 to make certain that any elders from your congregation who died in the past year are reported and honored during the Presbytery Stated Meeting on Tuesday, December 6, 2022.

Please enter the names of elders (active or inactive) in your congregation who died between October 1, 2021 and September 30, 2022.

If there were no elder deaths, state "NONE" instead of listing the names of elders so that it may be reported that your congregation did respond.

NOTE: This list is to include ordained RULING ELDERS only.
