To avoid using paper forms, this form replaces the British Cycling parental consent form and applies to Go Ride coaching sessions run by Thanet RC and/or VC Deal.

Please fill out a separate response for each child attending the sessions.

This data will be kept securely for 1 year after your child stops attending sessions, or until an updated form is provided. We will not share your data with any other parties EXCEPT if required to do so for Covid track and trace attendance.
