The Collins Collective Booking Form
Hi we're super excited you're ready to book with us!
In order to start your agreement and credit card authorization form we need some information:

Some information is required, while others are optional (3rd party for instance)
Please note - each person named in the form MUST have an individual email address. This is required by our software for signature security purposes. Each person WILL receive a copy of the paperwork in their email directly and must sign for the paperwork to be complete. You cannot sign for all parties through one email address.
Once complete it will be sent to our staff who will reach out with any questions. Please be sure to email admin@thecollins.co & sales@thecollins.co your 2 dates to ensure we received your form.
