Survey for Parents about Travel to and from School
Dear Parent or Caregiver,

Lawrence Public Schools, in partnership with the Lawrence-Douglas County Health Department and the Lawrence-Douglas County Metropolitan Planning Organization (MPO) want to learn your thoughts about travel to and from school for children in grades K-8. This survey will take about 5-10 minutes to complete. We ask that each family complete only one survey. If you have more than one child in grades K-8, please answer for the student whose birthday is closest to today's date.

Your responses will be kept confidential and neither your name nor your child's name will be associated with any results. If you have any questions, please contact Sarah Hartsig, shartsig@ldchealth.org

Thank you for participating in this survey!
