2019-2020 Lifespan Spiritual Education Registration for Children & Youth
Hello and welcome to the Unitarian Church of Montpelier!

Thank you for taking the time to register your child or youth. We are thrilled to have you be part of our community. We take the well being and safety your child(ren) very seriously. The information you provide on these forms enables us to better meet their needs along with your own needs as their caregivers. This form is for any kids or youth participating in our Lifespan Spiritual Exploration programs including:

-nursery

-Sunday morning programs for preschool-middle school

-Our Whole Lives or OWL

-High School Youth Group

-Youth who are on the Child Care Team (either for nursery or extra child care options).

If you would like to set up a visit to the church with your child or youth before you come for the first time, please email me at ucm.dre (at) gmail.com and I'm happy to arrange a time to give you a tour and talk through any questions you might have.

Reminders and Requests
If you have more than one child in the LSE program, feel free to fill out the sections below only once.
Nursery pick-up is right after service ends. For parents of children PreK-3rd grade: Please pick up any children in 3rd grade or younger by 10:20 a.m. (for the 9 a.m. service) or 12:20 p.m. (for the 11 a.m. service). NOTE: This is a slightly later time than years past. It gives you more kid-free time at Coffee Hour and helps get more into fun projects / activities with the kids. If on certain days you must pick your kid up promptly at the end of service, we can be flexible.
Children / youth are welcome to stay for all of worship in the sanctuary. Some prefer to listen with in the Fireplace Room. If your child is choosing not to participate in Sunday morning LSE programs, parents are responsible for their safety and proper supervision (varies by age and by child/youth so consult with Liza if you're unsure).
Important reminder: Some elders, babies and others at church have sensitive immune systems. Please don't bring your child(ren) or youth to the church if they have a fever, have been vomiting within the previous 24 hours, or have a communicable illness in the contagious stage (ex. Hand Foot Mouth disease). *

Ways You Can Support the Lifespan Spiritual Exploration Program
We are delighted to have you as part of our religious education family! It takes a lot of caring hands and hearts to run our program. Some churches have a cooperative model, where at least one parent/caregiver is helping most Sunday mornings. At UCM we don't have any mandatory level of participation, be we do hope that you’ll join in as you're able when volunteers are needed.
