We reserve the right to charge your credit card a reasonable and customary fee, $50.00 for missed appointments or services that were scheduled for you, this applies to new and existing patients.

1. Failure to call 48 hours in advance to cancel.

2. Failure to show for your scheduled appointment.

3. Arrive too late for the doctor to see you at your scheduled appointment time.

If you are recommended and scheduled for Neuropsychological Testing/ADOS testing and you either no show or cancel less than 48 hours your credit card will be charged a reasonable and customary fee of $100.00.
