Please send your registration fee of $50
per person as soon as possible. That one fee will cover the cost of a new Alumni Booklet and
allow you to attend all the of reunion events, including the Alumni Banquet, as
well as most of the musical performances planned by the Pollock Dairy and Ag Days committee and the Pollock 125th Celebration committee. NO REFUNDS will be given. Please let us know if you plan to
attend the Alumni Banquet meal so that we can plan the meal more efficiently!
Fill this form out ASAP, so that the
Reunion Committee can complete final details. After June 1st, the registration fee is $75 per person. You can Venmo your registration fee to PHS Alumni @PHS-Alumni.
As a word of warning, there may be websites/pages
that claim to be the “official” PHS Alumni website or Facebook page. The Reunion Committee has
no affiliation with any of these websites or Facebook pages, and any money sent to them will be
lost. You need to fill out this official form to get registered.

Make checks for your registration fee payable to the Pollock Alumni Association. Please write a separate check if you want to make a donation to the Pollock 125th Celebration in your class name. Please make that donation check payable to Pollock 125th Celebration and put your class year in the memo. Both checks can be mailed to the above address.
