Advisor responsibilities
- I understand that advisors must be present at all club activities
- I understand that it is my responsibility to oversee all club paperwork, field trip paperwork, and financial matters
- I understand that any volunteers working with the club must go through the district clearance process to working with students
Club responsibilities
To remain in good standing, I understand that this club must:

- Attend all monthly Club Council meetings
- Turn in at least two completed signed meeting minutes per month
- Turn in Club info packet in August
- Turn in club budget in September
- Turn in club budget monitoring in November and February
- Turn in club renewal in March
- Turn in club carryover form in April

Failure to complete the above activities may result in the probation and/or dissolution of the club.
Student and advisor approval of this form constitutes the acknowledgement of the above responsibilities.
