Informed Consent Statement
Please read carefully and contact me if you have any questions before agreeing to this form. Once submitted, you will receive a copy of this form.
(1) I am at least eighteen years of age, and voluntarily seek philosophical counseling from Max Parish. I am free to seek his services on a continuous or intermittent basis, or to discontinue them at any time.
(2) My identity as a client will remain confidential to the extent permitted by law. Exceptions include if Max Parish determines I am a possible danger to myself or others or becomes aware of possible danger to minors.
(3) I understand that philosophical counseling is not psychotherapy or clinical counseling, and involves only dialogue with a philosopher for educational purposes. Philosophical counselors do not diagnose, address, or treat mental health issues, nor are they a substitute for mental health counseling. By agreeing to this statement, I verify that I am not entering into philosophical counseling for mental health issues, and that I am not in need of mental health counseling or that I am receiving mental health counseling elsewhere.
(4) I understand that, as a client, I will be asked to discuss my issues with the philosophical counselor, and to answer relevant questions to the best of my ability.
(5) I understand that the philosophical counselor will make no decisions for me, nor direct my behavior. Instead, the philosophical counselor’s primary role is to help me clarify my understanding of the implications of my possible decisions and to sort out my own thoughts, beliefs, values, and preferences in connection with such matters.
(6) I understand the possible benefits philosophical counseling offers—for example, I may gain a deeper understanding of the issues we discuss, and I may become more empowered to handle and/or resolve the issues we discuss.
(7) No risks are expected to result from the philosophical counseling process, as it is an educational practice.
(8) Pricing: A 30 minute discovery call is $45. Individual sessions are $125 per 60-minutes. All sessions are paid in advance via PayPal or Stripe. Discounted rates may apply to sessions purchased as part of a package.
(9) All sessions are nonrefundable. Any session may be rescheduled up to 24 hrs prior to the scheduled session time. Sessions cancelled within 24 hrs of the session time cannot be rescheduled. For single sessions, the rescheduled session must take place within 60 calendar days of the original session date. Sessions purchased as part of a package may have a longer rescheduling window, depending on the package. Please reach out if you have any questions about rescheduling or cancellations.
(10) I have had the opportunity to ask questions about any of the above, and to ask preliminary questions about philosophical counseling. All questions that I did ask were answered to my satisfaction.
(12) While there are no known risks from philosophical counseling, from this date forward I knowingly and freely assume any and all possible risks associated with Max Parish’s philosophical counseling services, and I release and hold harmless Max Parish with respect to any and all harm, injury, physical or mental illness, disability, death, or loss or damage to person or property, whether arising from negligence of Max Parish or otherwise, to the fullest extent permitted by law.
