Self Care Quiz
How do we become that man who gives easily, from a seemingly bottomless reserve? How do we be that amazing father, spouse, partner, or friend? The process starts from the seemingly contradictory practice of self-care.

This survey is designed to support you in your fight to reclaim your life, and your authentic power through daily self-care practice. The first step in this fight is to take a fiercely honest assessment of your life, your current practices or lack thereof, and the impact. The answers you provide to these questions will highlight your strengths to be celebrated and challenges to take on. I encourage you to use this survey to reflect upon yourself and your own journey and work. The more honest you are with yourself, the more effective the tool. The questions are on a scale from one to five, but take time to write out answers to each one as well.

All answers are on a scale from 1-5. 1 being the least proficient, 5 being the highest.
