For good and valuable consideration, receipt is hereby acknowledged, I authorize Mary Joyce and Do Big Good LLC (“Producer”) to make use of video footage, photographs and audio recordings (“Footage”) of me captured by the Producer or her agents for #DoBigGood for America (the “Program”) and such related uses as may be determined by the Producer.

Such related uses may include, but are not limited to, electoral campaigns, news stories, activism, marketing, and use on websites and social media.

The footage and any resulting videos are subject to a Creative Commons Attribution-ShareAlike 3.0 License.

----------- Agreement -------------

I agree that you may use Footage of me, including images, voice, conversation and sounds without claim to:

1) Financial compensation (I will not be paid for my appearance in this Footage or Program.)

2) Right of inspection and approval (I will not have the opportunity to review how the Footage of my appearance is edited into the Program.  I understand that the Producer commits to edit that Footage, to the best of her abilities, such that it is a truthful representation of my statements and opinions.)

3) Ownership (I will not own the Footage.)

I understand that the personal information given below will not be shared with third parties and is collected only if the Producer needs to contact me in the future.
