Reporting A Lost Item
Please fill out this form to report a lost item at Music City Food and Wine Festival 2019. Filling out this form is the best way to report a lost item(s) so that if we locate your item, we can contact you quickly. We'll review all reports as quickly as possible and get back with you if we locate your item. We thank you in advance for your patience as we sort through these requests and identify owners.
Please Note: We will contact you ONLY if we locate your item. We are not able to contact every person to inform them their item was not turned in.
After the festival, please email
to inquire about lost items or arrange for pick-up/shipping of any found items that have been matched to your claim. Valuables will be held for 30 days and then donated to a local charity. C3 Presents will not be held liable for any unclaimed items.

Description of Items
- Driver's License/ID: Name and address on the license/ID and the state it was issued.
- Camera: Model & color, etc. (ex: Canon Sure Shot, silver, in black leather case with silver wristband) + describe recent photos, if any.
- Cell Phone: Model, color, case color and brand and passcode, if applicable (the more details, the better - this is the largest category of lost items). Example: iPhone 6 in white OtterBox rubber case with passcode 4321; Red Samsung flip phone with a cat sticker on the back; Black Blackberry with golden retriever as background photo.
- If your cell phone is lost, please leave an alternate phone number to reach you such as your work, home, or the number of someone who you regularly communicate with!
