To help us ensure your child’s safe and smooth transportation to and from school, please complete this form if your child is being picked up when they ordinarily ride the bus home, are staying after school of an activity, or other change to their normal way of getting home.

Depending on your selections, you’ll be asked to provide additional information such as bus numbers, pick-up contacts, or club names. This form helps our staff coordinate transportation and dismissal procedures efficiently and safely.

All responses must be submitted before 2:00 PM on the day of the change. We cannot guarantee that changes submitted after this time will be seen and processed in time for dismissal.
