CIAFV Membership Survey
AndersonDraper Consulting Inc. has been contracted to undertake an evaluation of Community Initiatives Against Family Violence (CIAFV). As a member of CIAFV, we are asking for your feedback. The information you share will be used to improve CIAFV and report on the strategic goals. Please note your participation is voluntary and responses will be summarized. Individual responses will not be shared, but some statements may be used as quotes to illustrate common themes.

This should take about 10 minutes to complete. To thank you for your time, you can enter a draw for one of three $10.00 Tim Hortons gift cards (your contact information will not be associated with your survey results).

If you have any questions about this survey, please contact the Evaluator, at 780-680-4195 or send an email to michelle.andersondraper@shaw.ca.
