Reinstatement Request Form
I am submitting the following request for reinstatement (restoration) into my former classification at AUHSD.

I understand that in accordance with Personnel Commission Rule 11.2.3, when a vacancy exists in my former classification, the appointing authority will choose from eligibles (from the eligibility list) as well as transfer and reinstatement eligibles. The appointing authority will hire the person he/she feels most closely fits the requirements of their school or site.

I also understand that if I am receiving a retirement allowance from the Public Employee Retirement System (PERS) that my retirement will cease on the date I resume permanent employment.
