Urey Student Schedule Request Change Form
Complete this form and submit to the school counselor.

NO CHANGES WILL TAKE PLACE WITHOUT COMPLETING THIS FORM AND FOLLOWING PROCEDURES.  Students are placed in classes based on academic level, teacher recommendations, and class availability.  Students were placed in elective classes based on student requests AND schedule availability.  Students may be in electives they did not select because of conflicts with other classes or availability in the class.

Changes in student schedules are made on the basis of the policy listed in the Urey Handbook, stated as follows:

Student is missing a core class.
Student is inappropriately placed based on grade level or academic skills.
Student has documented evidence supporting a course change.

REASONS OTHER THAN THE ABOVE WILL RARELY BE CONSIDERED.

1 WEEK (5 days) Rule for Electives...students have the first 5 days to request a change in electives.  Because of class sizes and availability, schedule changes may not be approved.  Absolutely no schedule requests will be considered after the one week period closes.
