By signing below, I acknowledge that I am aware of the risk of injury or illness inherent in any activity or program designed for young children. As the parent or legal guardian of the child above, I understand that SF Koreanuri Summer Camp is permitting my child to participate in SF Koreanuri Summer camp upon the express understanding that I hereby waive and release SF Koreanuri Summer Camp, its employees, agents, officers, and directors from any and all claims arising from my family's activities at SF Koreanuri Summer Camp, and indemnify and hold SF Koreanuri Summer Camp harmless from and against any and all liabilities, expenses or judgments, including attorneys fees and court costs resulting therefrom.

I acknowledge that SF Koreanuri Summer Camp does not have licensed medical staff available onsite and is not obligated to administer any medical care or medications. In the event of an emergency related to my child, I understand SF Koreanuri Summer Camp will use reasonable efforts to contact me, the named emergency contacts or named family physician. If SF Koreanuri Summer Camp is unable to reach me or my named contacts, I give permission to SF Koreanuri Summer Camp to seek whatever medical care the child needs as determined by a SF Koreanuri Summer Camp employee in her or his sole discretion and hereby release SF Koreanuri Summer Camp from all liability resulting therefrom. I further agree to be responsible for any and all charges for the emergency medical care.

Parent(s) or court-appointed legal guardian(s) must sign for any participating minor (those under 18 years of age) and agree that they and the minor are subject to all terms of this document, as set forth above.
