2017 Heroes Summer Academy Registration
Thank you for your interest in St. Mark Youth Enrichment's 7-week summer program hosted at our Dubuque site. The theme this year is Heroes Building a Better World. Aligning with the National Collaborative Summer Library Programs, weekly themes incorporate community service, social and emotional well-being, and health and wellness into our Literacy and STEAM (Science, Technology, Engineering, Arts, Math, and Music) curriculum. Program includes a daily lunch and snack, provided by the Boys and Girls Club, as well as an opportunity for breakfast in the morning. Students will also receive a t-shirt and 7 books to keep by the end of the program. Students entering grades K-5th in the Fall of 2017 are eligible. The program will run June 19th-August 3rd. Morning and afternoon offerings depend on the grade level your child will be entering, and this year, full day options are available through our partnerships with the Boys and Girls Club and Leisure Services. To complete your registration: 1. Complete this registration form before April 1st. 2. Pay any outstanding program fees. 3. Pay the non-refundable $25.00 PER FAMILY registration fee. 4. Attend Parent Information Session by May 31st. 5. Make arrangements to pay tuition fee. The registration fee is PER FAMILY, and is different than the tuition fee, which is income based. If there are no outstanding balances from a St. Mark program, the registration fee is received, and the form is completed, you will be contacted by the St. Mark administrative team regarding the status of your registration. When approved, a mandatory parent information session is required for your child to be fully enrolled and attend program. The tuition fee, per child, is $250 for the 7 weeks with full to partial scholarships available. You will select a payment arrangement option further in this form and an invoice will be generated and mailed to you based on your selection. No family will be turned away because of inability to pay program fees! Please read all the information carefully and completely.
