If you would like to receive notification before pesticides are applied to school sites please complete this form.

FYI- Certain types of reduced-risk pesticides are exempt from the notification requirements. These reduced-risk pesticides include:
• Self-contained baits or traps
• Gels or pastes used for crack-and-crevice treatments
• Antimicrobials
• Pesticides exempt from registration under the Federal Insecticide, Rodenticide, and Fungicide Act (FIFRA)
