Withdraw Notification Form
I am requesting the following student/class enrollment be withdrawn from Ridgewood Gymnastics. As per the written policy of Ridgewood Gymnastics and the City of Dallas, I further understand the following:

1. All students enrolled require this notice of withdraw form for any Ridgewood Gymnastics program. This notice must be received by the 25th day of the month prior to the month you wish to withdraw your student, in order to avoid additional tuition charges. Any notice received after the 25th day of the month will have full tuition charges for the following month assessed.

2. You are responsible for payment of your student’s classes WHETHER OR NOT YOUR STUDENT ATTENDS CLASS until the time you submit an approved withdraw request. There is no tuition pro-ration for holidays or withdrawing before the end of the month. Classes continue on a year round schedule.

3. All withdraw notices will receive a confirmation email. I understand it will be my responsibility to ensure my withdraw notice is properly received by Ridgewood Gymnastics and the City of Dallas, and that I have received a CONFIRMATION EMAIL. However, please note we DO NOT EVER accept withdraw notices via phone, fax, verbally, or via regular mail.
