以下をご確認の上、エントリーをお願いいたします。
This will be the entry form of 【18th Cooperation Cup】.
Please check the following and enter.

【新型コロナウイルス感染防止への対応について】
下記に該当される参加者・見学者の方はご入場いただけませんので予めご了承ください。
・会場内でマスクを常時着用いただけない方
・会場入場時の検温時に発熱（37.5℃以上）がある方
・のどの痛み、頭痛、関節痛、息苦しさ、息の吸いづらさ、咳、下痢、
結膜炎症状、味が薄く感じる、味覚障害などの症状をお持ちの方
・新型コロナウィルス感染症陽性と診断されてから14日以内の方
・新型コロナウィルス感染症陽性とされた方との濃厚接触してから14日以内の方
・新型コロナウイルス感染症PCR検査・抗原検査等の診断待ちである方
・同居のご家族や身近な知人に感染が疑われる方がいらっしゃる方
・2週間以内に政府から入国制限、入国後の観察期間を必要とされている
国・地域への渡航、ならびに該当国の在住者との濃厚接触がある方
※政府・各自治体による要請により、上記内容が変更になる可能性がございますので、
その際には改めて告知させて頂きます。
「Regarding the Preventive Measures of COVID-19」
The following people will not be allowed to enter the venue:
-People who are not wearing a mask inside the venue.
-People who display a temperature above 37.5℃ upon entering the venue.
-People who are suffering from Sore Throat, Headache, Joint Pains, Shortness of Breath, Coughing, Diarrhea, Conjunctivitis, Loss of Taste, Dysgeusia, etc.
-People who have tested positive for COVID-19 in the past 14 days.
-People who have had close contact with someone who tested positive for COVID-19 in the past 14 days.
-People who are waiting for the results of a COVID-19 PCR test.
-People who live together or spend time together with someone suspected of having COVID-19.
-People who have traveled to or had close contact with a resident of one of the restricted countries for entry into Japan in the past 14 days.
*There is a chance these may change based on new information from the government; in that case we will make another announcement.
