The undersigned participant and/or his/her guardian, in consideration of the Sarasota County Government and the Englewood Sailing Association, Inc. and any of their members providing facilities, instruction, and supervision in the area of sailing instruction do herby:
1. Assume all risk of possible danger or injury involved through participation in noted activity.
2. Request permission to participate in the activity with full knowledge that activity could result in damage or injury.
3. Agree to indemnify and hold harmless the Sarasota County Government and the Englewood Sailing Association, Inc. from liability resulting from participation in said activity.

Visual/Audio Image Release
I grant permission to Englewood Sailing Association, Inc. (ESA), its employees and agents, to take and use visual/audio images of me or my minor child(ren). Visual/audio images are any type of recording, including photographs, digital images, drawings, renderings, voices, sounds, video recordings, audio clips or accompanying written descriptions. I agree that ESA owns the images and all rights related to them. The images may be used in any manner or media without notifying me, such as web sites, publications, promotions, broadcasts, advertisements, posters and theater slides, as well as any other uses by ESA. I waive any right to inspect or approve the finished images or any printed or electronic matter that may be used with them. I release ESA and its employees and agents, including any firm authorized to publish and/or distribute a finished product containing the images, from any claims, damages or liability which I may ever have in connection with the taking of use of the images or printed material used with the images.

My typed name below acknowledges that I have read this Sailing Program Liability Waiver and Video/Audio Image Release, fully understand their terms, understand that I have given up substantial rights in signing, and sign freely and voluntarily without any inducement.
