2024-2025 Official Notification of Withdrawal from Minnehaha Academy
This form shall serve as Official Notification of Withdrawal for the child(ren) named below from Minnehaha Academy.  REMINDER:  Our Board policy states that all student accounts must be paid in full before official records, grade reports or transcripts can be released.  Refunds, when applicable, will be processed in accordance with Minnehaha Academy's Tuition Refund Policy.  NOTE:  All iPads (including all accessories), books and other school-owned property must be returned to the school office before a refund will be processed.   Additionally, all emails, data stored in Google Drive, and logins associated with the student's school account will no longer be accessible after the withdrawal date.  

Any reservation deposit already paid is non-refundable.
