2023-24 LBSS Transcript Request Form
Complete this form after you have submitted your Consent for Release of Student Records in Support of Postsecondary Applications (IS-111). The Consent Form only needs to be completed 1 time. 
Fill out this form for each college/scholarship/transcript request. 

Initial Transcripts will be sent the week of September 11th.  

Counselors will start writing recommendation letters after senior meetings, which will occur mid-September.

Request deadlines are as follows:
