Health Awareness Attestation
As an act of love and compassion toward myself and my community, I want to always remain aware and intentional about how my actions impact the health and well-being of my community, my family, and myself. To assure that I am always as safe as possible for any interaction I may encounter as I enter the church building or whenever participating in outdoor church sponsored functions, I attest that:
I do not have a fever today greater than 100.4 degrees F. I took my temperature before leaving home.
I am feeling well, with no symptoms of cough, sore throat, headache, new shortness of breath or difficulty breathing, chills, muscle pain, nausea or diarrhea.
I have not been in the presence of a known positive Covid-19 patient within the past 2 weeks.
I will wear a mask in the presence of others and practice social distancing at all times.

This attestation needs to be completed each day that a person enters the church building or whenever participating in outdoor church sponsored functions until the Covid-19 pandemic is declared controlled. It shall be filled out when entering the building by hard copy to be left in the folder at the east door, or on-line. The purpose of this policy is to assure that each person has done an intentional self-assessment of his/her health at the time of entering the building or attending church sponsored functions.
