I understand that Palm Beach Pet Hotel and Spa requires my written permission in order for my dog to be allowed to freely come in contact, socialize, play, and interact with other dogs, and I hereby give that permission effective from this date forward.

VACCINATION POLICY
I understand that we require proof of immunization on file. I certify that my dog is current on all vaccinations in accordance to hotel policy and I will provide the necessary documentation to prove this prior to each visit. I also understand, the hotel requires all services be updated no later than 14 days prior to due date to ensure maximum protection. In the event that I am unable to provide proper documentation, I will not be allowed to leave my dog for the day without proper services being performed. Should I leave my dog and it is later determined they are overdue for required services, these services will be performed and I will be responsible for the charges associated with them.

CONTAGIOUS ILLNESS POLICY
In the event that my pet contracts anything viral or bacteriological, I understand I will be unable to bring my dog back to daycare until they have been medically cleared by our veterinary staff. I also understand that I will be financially responsible for the treatment of my pet, I also understand that I will be responsible for any illnesses that may develop in the future, as a result of, or in any way relating to Palm Beach Pet Hotel & Spa and Baker Veterinary Clinic.

ACCIDENT/INJURY POLICY
I understand that injuries and or damages may occur. In the event of injury or damages, I understand that any treatment will be at my expense and neither Palm Beach Pet Hotel and Spa or Baker Veterinary Clinic are financially responsible for any injuries or damages of any kind to pet or property, and also any and all injuries and damages that may develop in the future, as a result of or in any way relating to Palm Beach Pet Hotel and Spa or Baker Veterinary Clinic.

GENERAL RISK POLICY
I understand, it is my responsibility to notify our staff of any medical problems or behavioral changes/abnormalities within the last 30 days immediately.

I understand Palm Beach Pet Hotel and Spa puts safety of all pets before anything else. I do, however, understand, the risks associated with daycare and accept full responsibility for complications.

I release Palm Beach Pet Hotel and Spa and Baker Veterinary Clinic, its employees, successors, and assigns, and their respective heirs, personal representatives, affiliates, successors and assigns, and any and all persons, firms or corporations liable or who might be claimed to be liable, whether or not herein named, none of whom admit any liability to the undersigned, but all expressly denying liability from any and all claims, demands, damages, actions, causes of action or suits of any kind or nature whatsoever, which I now have or may hereafter have, arising out of or in any way relating to any and all injuries and damages of any and every kind, to both pet and property, and also any and all injuries and damages that may develop in the future, as a result of or in any way relating to of any liability arising from my dog's attendance and participation in activities.

This agreement has no expiration with the exception of when it is replaced with an updated version provided by Palm Beach Pet hotel and Spa at Baker Veterinary Clinic.
