The U.S. Department of Education (USDE) has requested data pertaining to the ESSER I, II, and III uses of funds during the FY 2021-2022 fiscal year at the state, district, and school level. This survey concerns how ESSER school-level allocations were determined. For USDE reporting purposes, please fill out this form and submit by March 30th, 2023.
