CHS/LVUSD Concurrent Enrollment Request Form: Summer 2026
This form is for SUMMER 2026 Concurrent Enrollment Requests only.

NOTES: Requests take our team ample time to review - please know, that at the end of the month (after your submission) a Counselor or Administrator will email the student and parent at the email addresses you provide below when your request has been reviewed & approved/denied.

CHS Credit is not guaranteed for outside coursework until reviewed & approved by a Counselor/Administrator. Concurrent Enrollment courses must be taken through an accredited institution and approved for college eligibility with the UC/CSU system.

Summer School Concurrent Enrollment Requests will no longer be accepted after Friday, June 19, 2026. The deadline for the completion of courses is Friday, August 14, 2026.

If permitted, student needs to arrange to take a LVUSD-approved final exam by contacting the department chair and pass the final with a “C” or better. It is the student’s/parent’s responsibility to verify college or NCAA acceptance. *
This form must be completed, and approved PRIOR TO the start of instruction. This form is to be used for requesting permission to enroll in: An Out-of-District class through an accredited school or program; an approved online class;  OR a two-year and/or four-year college class *
I understand that district drop deadlines are still in effect. District guidelines allow students to enroll in specific classes that may be included on the student’s transcript; may meet certain graduation requirements; may be used to extend or accelerate a student’s academic program; and/or may be used for make-up credit. *
