FASA Common Area Reservation Form

In order to have time to process reservation requests for weekend reservations all reservations for use of FASA Common Areas must be completed 72 hours prior to the day you wish to use space with the exception of the use of the piano. Piano reservations can only be submitted 1 day prior to the day you wish to use the piano.

FASA Common Areas are made available for FASA residents only. Therefore, a FASA RESIDENT MUST reserve the area, check out the key (if needed), be present the ENTIRE time, and check-out with the receptionist on duty. FASA Common Areas are reserved on a first come first serve basis using this reservation form.

FASA Common Areas cannot be reserved for recurring activities. Residents may reserve each common space up to two (2) times per month. FASA Common Areas may not be reserved for religious purposes unless it is open to the entire FASA community.

The area will be inspected at the conclusion of the reservation (or the next business day if after hours). Incomplete Cleaning will be assessed at $30 per hour for Housing Staff to clean, with a minimum 1 hour charge. Damages are charged at the Labor Hourly Rate ($30 per hour) plus the cost of materials. The Aggie Village Community Center (AVCC) Key must be returned within 24 hours of checking it out or a $5 per day late fee will be assessed. If the AVCC Key is not returned within three days or gets lost then the responsible party will be charged the cost of rekeying the community center. A charge $30 per hour will be assessed if the office staff has to stay past closing to wait for you to finish any duties. All charges will be placed on the resident's Housing Account.

To better serve you, we have published the AVCC Reservation Calendar and the FASA Pavilion Reservation Calendar on our Community Website for your convenience. By reserving FASA Common Areas, you give consent to FASA Staff to place your name on our calendar when completing the reservation.

    Terms & Agreements

    I have read, and agree, to follow the guidelines and cleaning procedures. I understand that failure to do so will result in charges of $30 per hour for cleaning, the cost of any damages to the Common Area I have reserved, and charges for not checking out on time will be placed on my housing account. I further agree to hold harmless Utah State University, its officers, directors, partners, agents and employees from any damages to property or injuries which I may suffer due to my use of Utah State University facilities and/or equipment. I agree to release and hold harmless Utah State University and its agents, against any claims, demands, actions, debts, liabilities, or judgments, costs or attorney fees arising out of claims on account of, or in any manner predicated upon, my use of Utah State University's facilities and/or equipment including loss or damage to property, any injury or death of any person in any manner, caused or contributed to by Utah State University, its officers, or its agents.
