This survey should take about 15 minutes to complete.

Individual responses to this survey will not be shared with anyone and will only be used as described in this survey’s introduction. Aggregated data and survey findings may be used for communications' pieces or reports and shared with local agencies and organizations working to improve services, approaches, healing practices for persons, families, and communities. Any information that could identify you will be removed or changed before being used in communications pieces or reports. 

You will have the opportunity to receive a gift card if you choose to fill out this initial survey and then the follow up survey at 45 days post initial response, at the end of this survey we will give you the details.
