LMLA Feedback & Incident Report
PLEASE READ: Use this form to provide feedback and report incidents of concern directly to the Langley Minor Lacrosse Feedback and Discipline Committee. The Form can be submitted anonymously, however, our preference is that you provide your name and contact info so we are able to follow-up and ask further questions if necessary. We recognize some types of feedback maybe difficult to provide, however, it is critical for the health, safety and positive experience of our youth athletes and membership in general that concerns be heard.

At the discretion of the Discipline Committee, your report may be shared with appropriate Board Members to gain a balanced perspective and determine course of action with a focus on what's right for our youth athletes. You can expect an initial reply from the Discipline Committee within 3-5 days of your report submission.

Alternately you can directly contact our Discipline Committee at discipline@langleythunder.ca. The Discipline Committee currently consists of two Executive Members, Serina Rowe (Secretary) and Wendy Thomson (Director, Administration).

* You must immediately report issue of risk of harm to a person and/or that are potentially criminal in nature directly to the local police by calling 9-1-1 or the local non-emergency number. As a secondary step submit an incident report to the LMLA Board *
Code of Conduct & 24 Cool Down Hour Policy *
Code of Conduct applies to feedback and incident reports and as such shall be accurate, truthful, constructive and provided in a respectful manner no matter the situation. If you are "heated" or over the incident you have cooled off for 24 hours before submitting.
