Use this form to tell HFSD whether your chapter is interested in a chapter Venmo or Cash App account for Humanity First fundraising. Submitting this form is a request for review by HFSD and Humanity First USA. It is not an approval.

Please do not open, link, or use any payment account for Humanity First funds unless HFSD has approved your chapter in writing. Until then, and even after approval, your official Humanity First giving page remains the default way to collect every gift, because it gives each donor a receipt.

Do not enter account usernames, passwords, bank details, or card numbers in this form. HFSD will contact your chapter if more information is needed. The form takes about five minutes.
