Singularity Psychiatry New Patient Request
The purpose of this form is to let us know more about prospective patients, what care is being sought, and let us get started with registration.

During regular business, this form is checked for responses daily. Please give us up to two business days to respond. In general, we will hold your place on the waitlist for 3 business days starting on the day of contact; if we don't hear from you within that timeframe, we will move to the next patient on the list. 

We will contact the prospective patient directly to start registration and, if there are openings currently available, schedule. Typically, new patients are scheduled up to eight weeks in advance, and we offer slots weekly as they become available. If no openings are available at initial registration, names are placed on a waiting list in the order we were contacted.

Patient Focus
Dr. Toups is an expert in treatment resistant depression, and more generally, in complex mood disorders, especially in patients with medical comorbidities. 

Patients meeting those criteria are given special consideration for treatment at Singularity; however we also consider patients with general psychiatric needs as long as we have openings. 

Unfortunately we can't accept all requests, and completion of this form does not establish or guarantee an ongoing doctor-patient relationship between Singularity Psychiatry and the prospective patient. We will give you an estimate of the wait time when we contact you and can often suggest resources to consider if we are unable to accept you as a patient or wait times are very long.
