This is a survey designed to help us help future amputees during the difficult and long post-op rehabilitation.The aim of this survey is to allow us to communicate directly with the patients and gain insights as to how the patient feels about the process. 

By answering the following questions (24 multiple choice questions, estimated 5-10 minutes to complete), you can help future amputees complete their rehabilitation more quickly and with fewer difficulties. 

Our survey is aimed mostly at lower limb amputees, but feel free to answer if you know or care for any such person.

The data collected will be used for our research purposes only and includes no identifying information. This research is being done by students of the Ben Gurion University in the Negev.
