Basic information collected on potential volunteers for the 2020 US Nationals hosted by Michigan Tech January 2020. If you are under 18 you must also submit a parental consent form: https://www.mtu.edu/research/administration/integrity-compliance/docs/volunteer-service-parental-consent-agreement.pdf

These forms can be emailed to Stef Cook or turned in the day of your volunteer assignment.
