VERY IMPORTANT, PLEASE READ FULLY BEFORE COMPLETING FORM: Each participant desiring to attend LTC 2019 MUST complete this online registration form. Each participant must have campus minister approval. Before you register, talk to your campus minister about any questions/clarifications. Unstaffed campuses will be approved by the designated person serving as the core coordinator for the current year (2018-19).
When you finish answering all the entries on this form, you will confirm that you have submitted A COMPLETE REGISTRATION by:
1. Responding to all information for this online form
2. Choosing a registration fee payment and mailing any necessary materials outlined in this form for that choice. NOTE: There are no part-time payments.
3. Downloading an "Oaks Liability/Medical Release" from ltc.aacf.org to complete and send to: ATTN: LTC 2019, JEMS, 948 E. Second St., Los Angeles, CA 90012.
4. Reading all necessary information related to LTC including registration, transportation, and cancellation.
BY CREDIT CARD/ONLINE at http://www.aacf.org/ltcpayment ** (VERY IMPORTANT - DO NOT CHOOSE TO PAY THE TRANSACTION FEE UPON CHECK OUT - FEE IS INCLUDED IN THE AMOUNT BELOW) Early Bird Registration: $415 - up to May 15, 2019 (8:59 pm, PST)Regular Registration: $440 - AFTER May 15, 2019 (9 pm, PST) and no later than May 31, 2019.
CONFERENCE GROUP PHOTO: If you would like to purchase a conference photo, the fees are an additional $10.00.
**NOTE: If your registration is incomplete or posted marked after May 15, 2019, you will not qualify for Early Bird Registration Fee. NO REGISTRATIONS will be accepted after May 31, 2019.
2) NO Cash or VENMO payments will be allowed. We ONLY accept check, money order, bank check as payments. You can pay by credit card through the online payment service (http://www.aacf.org/ltcpayment).
3) Registration costs vary according to (1) payment method, and (2) payment date. See the section above.
4) For partial registrations (i.e. scholarship), ONLY CHECK/MONEY ORDER/ELECTRONIC BANK PAYMENTS accepted and made payable to "AACF". Be sure to include "LTC 2019 Registration" in the memo line. Your name must be printed or written on the top left hand corner of the check. No partial payments by credit card.
If the check and/or the payment is for more than one person, please include a note with your payment that has the first and last name(s) of the persons being paid for (include payment amounts if paid by someone other than yourself or for multiple people).
5) CREDIT CARD payment is available ONLINE ONLY if paying the FULL AMOUNT. The transaction fees are INCLUDED in the registration cost, so do not add extra funds when prompted to add fees.
