CHILD MUST BE 3 YEARS OLD BY JULY 31, 2026 TO QUALIFY FOR PRESCHOOL

Please bring a copy of your child's:
  • immunization records
  • birth certificate
  • proof of residency in the LR school district (Approved items are: most recent city water or electric bill or a copy of your current lease agreement)
*If your child has previously been screened and you have already provided their birth certificate and immunization records, you do not need to bring these documents again unless the information has changed or the records have been updated. However, you are still required to provide a current proof of residence.

Due to limited space, we kindly ask that only one parent or guardian accompany the child being screened. Please do not bring siblings to the screening appointment.
