Students may submit this form to request to WAIVE .5 PE High School credit, according to WAC 180-51-068 and RCW 28A.230.050. Students must meet the following criteria to be eligible:

1) Student and parent/guardian must review and be in compliance with RCW 28A.230.050 & WAC 180-51-068,
2) Student must have completed Health, a CKSD graduation requirement
3) Student must have completed a full season of an OHS sport during grades 9-12. (This is a one-time credit waiver; students cannot waive more than .5 PE credit for multiple sports).
4) Student must receive school approval from the appropriate staff members indicated on this form.
5) Note: if approved, the .5 PE credit is NOT 'earned'; rather it is WAIVED, allowing approved students to take a different .5 credit in it's place.

Complete this form to submit your request, which must go under the review/approval process before it is accounted for in PowerSchool.
