3. Opt-in via this Form (ensure all information is accurate). Only those users who ticked the consent box and answered the 'customer details' section of the form will be part of the e-raffle.

4. If you agreed to participate through this form, your e-raffle entry will be credited within 1 to 2 business days after you submit it.
