As we do each year, we are reaching out to parents and guardians to share important documents and to ask for them to confirm that they have received some. In an effort to streamline our processing of these documents and to reduce paper consumption in our building, we have moved as many of these paper-based forms to the online form below. IT IS IMPORTANT THAT ALL PARENTS/GUARDIANS COMPLETE AND SUBMIT THE FORM BELOW by the end of the day on Tuesday, September 29.
If you have multiple children enrolled at ARHS, please complete the form multiple times, one for each child.
Students over 18 have the right to fill out this form for themselves and may use their school email to do so.
