By filling out this Online Library Orientation Request form you hereby allow/ authorize University of Perpetual Help Molino Campus and their authorized personnel to gather and process your personal information (name, address, contact number, email address and picture if applicable for documentation use) specifically for the purpose of gathering information on online learning capability of students. All information gathered in this survey form will be kept secured and confidential for a period of 2 years and will be destroyed by means of file deletion after the prescribed retention period in accordance with Republic Act 10173 or the Data Privacy Act of 2012 of the Republic of the Philippines.
