The Prior Lake Summer Baseball program asks that EVERYONE attending the ballpark for a Prior Lake Senior Team or Junior Team games at Memorial Park or Ponds Park should complete this online form prior to or upon arrival at the ballpark for EACH game IN PRIOR LAKE. Parents/Guardians attending need to fill out multiple forms, one for themselves and one for each minor they are signing for. This form applies to anyone who enters the ballpark (players, coaches, umpires, fans, PA announcers, scoreboard workers, concession workers, etc). Completing the form prior to arrival at the ballpark will speed up the process upon arrival and minimizes contact at the ballpark. We ask that you show the completed form, that will be emailed to you as proof of completion, upon arriving at the ballpark. Please be sure the person verifying check-in can see your name and the game date on the email with the completed form. If you do not show proof of completing this form, you will be asked to sign-in on the paper sign-in sheet for the safety of yourself and others at the ballpark.

If you have answered yes to any of the symptoms listed above, please do not enter the baseball facility. Please go home and isolate yourself from others and contact your doctor for medical advice.

By completing this form, I sign that I am not experiencing any symptoms of COVID-19. I accept all risks. The City of Prior Lake, Prior Lake Amateur Baseball Association (PLABA) , Prior Lake Summer Baseball Teams (Senior Team and Junior Team) and its coaches and administrators are exempt from liability in the event of COVID-19 illness. I will follow all appropriate safety guidelines at this event. Legal guardians of minor children must sign in their place.
