Please complete this form once for each person you are registering for the Convention. Your responses will be sent to the E-mail address provided below. (Please note that this system does not allow multiple registrations on the same form, but you will be provided with a link to submit additional registrations after completing the last step for this registration. This is how you will register more than one person for the Convention.)
The total fees for a person's registration, board dinner, box lunch, and banquet, will be tallied automatically. You will also receive a copy of information you provided in an E-mail message.
You have the option of paying online with a credit or debit card, or by mailing a check. You won't have to decide how to pay until the very end of the registration process.

- Registration: $20/person
- Board of Directors Dinner: $15/person
- Saturday Banquet: $30/person
