***All requests must be submitted and APPROVED by admin prior to completing this form***

- All requests forms and supporting documents must be submitted at least ONE WEEK IN ADVANCE to the next regularly scheduled PTO meetings.

- Submit all supporting documents via email to PSRESPanthers@gmail.com or leave it in the PTO mailbox in the Main Office.

- Please note that PTO will not distribute cash under any circumstances.

- Please be prepared to respond to any follow-up questions and be able to submit any necessary documentation the PTO may request before approving your request.
