This form will be used to document attendance anytime we are using an eLearning or Virtual Learning Day at Nativity of Our Savior School. Please take a moment to fill out all of the information below before you hit submit. IMPORTANT -- IF THE FORM IS NOT COMPLETED AND SUBMITTED YOUR CHILD WILL BE MARKED ABSENT FOR THE E-LEARNING DAY.
